A Community Health Network is a group of people from a wide variety of sectors that come together to promote health and well-being in their communities. The network serves as a platform to identify, prioritize, and take action on local factors to positively influence the social determinants of health. Social determinants of health are key factors that influence health and well-being. They include income and social status; social support networks; education; employment/ working conditions; social environments; physical environments; personal health practices and coping skills; healthy child development; biology and genetic endowment; health services; gender; and culture. This event is an exciting opportunity for people to come together and identify actions in our community that can be strengthened through collaboration.
